EB Medical in Rutland has graciously agreed to accept all of Dr. Strauss’s patients who have not already been connected with NP Maxwell or found a new provider.

Where are my records going?

EB Medical will become the custodian of ALL medical records associated with Dr Strauss once the practice closes on March 15, 2026. The College of Physicians and Surgeons requires medical charts/records to be stored for 16 years.


What if I already have a new provider?

You can start seeing your new provider right away and you will no longer be able to book an appointment with Dr. Strauss.


How do I request a chart transfer to my new provider that is not NP Maxwell or associated with EB Medical?

Visit the new provider’s office to sign a chart release form. The new office must fax the form directly to our clinic BEFORE March 12. The staff will process the transfer and send your chart to your new provider’s office. The patient is responsible for covering the fee of this uninsured service.



I’m happy to stay with EB Medical. When will my chart be transferred there?

There will be a data migration within 3-6 months after the March 15 closure. Both clinics operate on the same electronic medical record, and this type of transfer will be the most seamless way for patient information to be sent to EB Medical.


Immediately. Note, you will no longer be able to book with Dr Strauss if your chart has been transferred to another clinic.


When should I start seeing my new provider?

When can I book an appointment with EB Medical?

I need an appointment urgently. Dr Strauss is booked, and EB Medical can not see me in a timely fashion.

You can continue to see Dr Strauss until the last clinic day, March 12. If he has no available appointments, you may phone EB Medical to book.


Chart transfers to NP Maxwell can be processed through Dr Strauss’s clinic up until March 12, 2026. Sign a chart release form in office. The patient is responsible for covering the fee of this uninsured service. *For patients whose meet and greet is scheduled AFTER March 12, 2026, chart transfer requests will be processed through EB Medical after the data migration, 3-6 months after March 15, 2026.

As always, walk-in clinics, the Urgent Primary Care Centre, and the emergency room are available for acute care needs. For prescription emergencies, pharmacies can issue you an emergency supply of medication if you have run out.
